Concussion Policy

The Waza FC Spring Friendlies adheres to the Michigan State Youth Soccer Association (MSYSA) guidelines for tournaments as described below.

CONCUSSION POLICY:
- All MSYSA coaches must be able to provide their CDC Heads-Up Concussion Certification.
- All Non-MSYSA coaches must be able to provide the CDC Heads-UP Concussion Certificate or the NFSH Concussion in Sports Certificate.
- Teams must be able to provide the Michigan Department of Community Health/CDC Parent and Athlete Concussion Information Sheet for each player attending the tournament.

This means that ALL teams, both in Michigan and out of state, must provide a copy of the Concussion Information Sheet for each player, signed by the parent and player. Please keep the original with your team documents at the field, along with the players' medical release forms. The Concussion form is available on the website at wazafriendlies.com.

Register for the Waza Friendlies

WELCOME TO THE 2017 WAZA SPRING FRIENDLIES!

We are very excited to have your team compete in this year’s tournament!
Before your team can participate, you must register your team for the tournament.

STEP 1: SEND REGISTRATION DOCUMENTS. Registration will be done via EMAIL or by MAIL this year.
You will receive an email confirmation once your documents are received and everything is correct.

 EMAIL Option - MUST BE RECEIVED BY APRIL 5TH
• Please email scanned documents to: tournaments@wazafc.com
• Subject line must include GENDER, AGE, TEAM ID (i.e., "Girls U10 WAZASPRING1234554321")
• Send as one continuous file, preferably a PDF file. Do not attach each document separately.

 MAIL Option - MUST BE RECEIVED BY April 5th
• Please mail to: Waza Friendlies, 14424 Garden , Livonia, MI 48154
• Do not send originals, send copies only
Please send copies of the items listed below to register your team:
1. Roster stamped by your affiliated league
2. Player Passcards (front and back) for each player on your roster including guest players and coaches
3. Concussion Certificates (coaches/managers) and Information Sheets (players)
4. Liability Release Form properly signed and dated (on website at wazafriendlies.com)
5. Guest Player roster, passcard, concussion and medical release form (if applicable)
6. Risk Management Cards (coaches and team representatives)
7. Contact Info Sheet (on website at wazafriendlies.com)
* Medical Release Forms (do not need to be sent in, but must be with you at the field in case of injury)
Canadian Teams - We realize you have different forms and requirements. Please send copies of your roster, pass books and travel forms.

STEP 2: CHECK IN. Even though teams are pre-registered, a representative from each team MUST CHECK IN to pick up your approved roster and wrist bands at one of the following locations:
• Thursday, 4/20/17 from 6pm-9pm at Total Sports-Novi (41550 Grand River Ave, Novi, MI 48375)
• Friday, 4/21/17 from 6pm-9pm at Splash Universe (100 Whitetail Drive, Dundee, MI 48131)

Guest player rosters, passcards and medical release forms for Michigan players can be obtained by going to the MSYSA website at www.michiganyouthsoccer.org (Membership Services/Guest & Player Forms) or by calling MSYSA at 734-459-6220. Out-of-state teams should contact their state association about obtaining these forms.

Michigan coaches and team representatives can apply for Risk Management cards at www.msysa.net or by calling MSYSA. This is a state requirement. Team officials from other State Associations must provide their respective State Association's risk management documentation.

IMPORTANT: Players are only allowed to play for ONE team in the tournament. No player will be allowed to play for more than one team regardless of age group, gender or team. Teams that do not follow this rule will be disqualified. No double-rostering allowed!

Team Selection
Team acceptance to the WAZA Invitational Tournament will be completed by the "Selection Committee". Notifications are sent to all teams on a rolling basis. The selection process is assisted by the information provided to the tournament on your application- example - team records, level of play, out of town/state vs local teams (to achieve a balance), performance in post-season tournaments including the "Waza Invitational", etc. It is important to provide as much information as possible to assist the committee in properly evaluating your team. Cashing of the entry fee does not indicate acceptance to the tournament.

Accomodations
The WAZA Invitational has teamed up with its official housing service, Traveling Teams, to bring your team the utmost service for its housing needs. This will help make the process of reserving rooms easier and more reliable. If you wish to pariticpate in the WAZA Invitational, you must use one of the hotels provided by Traveling Teams.

Sadly, the Waza family has lost a dear friend and coach,

Sadly, the Waza family has lost a dear friend and coach, Seamus Rustin. Seamus passed away late yesterday (3/2/2015) of a heart attack at the age of 38 just after he finished coaching the game he loved. He is survived by his loving family – his wife Regilene and their three children Silas, Mila, and Lucian.
